Summary
Independently assess, diagnose and treat patients within scope of practice Manage patients presenting with acute and chronic conditions Undertake home visits where clinically appropriate Prescribe as an independent prescriber and undertake medication reviews Request, interpret and act upon investigation and pathology results Deliver chronic disease management and contribute to QOF and enhanced services Promote health, wellbeing and disease prevention Ensure safeguarding responsibilities for adults and children are met Maintain accurate and timely clinical records using SNOMED coding Work collaboratively with the multidisciplinary team Contribute to clinical governance, IPC and medicines management Support, supervise and mentor nursing and clinical staff Supervise students and learners where appropriate, supporting their development Develop and lead on an area of specialist clinical interest Support service development in line with national and local priorities
