Summary

Job Purpose The Primary Care Clinical Pharmacist will play a key role in supporting the delivery of safe, effective, and patient-centred medicines optimisation within general practice. Working as part of a multidisciplinary team, the post holder will contribute to medication management, structured long-term condition reviews, clinical audits, and quality improvement initiatives, supporting improved patient outcomes and prescribing safety. Main Duties and Responsibilities Work as an integral member of the multidisciplinary primary care team to deliver high-quality, patient-centred medicines optimisation services. Undertake structured medication reviews, including for patients with long-term conditions, polypharmacy, frailty, and high-risk medicines, in line with national and local guidance. Support the management of long-term conditions such as hypertension, diabetes, asthma, COPD, and cardiovascular disease, ensuring safe, effective, and evidence-based prescribing. Identify and resolve medicines-related issues, including adverse drug reactions, interactions, non-adherence, and inappropriate prescribing. Provide clinical advice and support to GPs, nursing staff, and other members of the practice team on medicines management and prescribing queries. Implement and support medicines optimisation initiatives in line with NICE guidance, local formularies, and ICB policies. Contribute to the delivery of clinical audits, quality improvement projects, and prescribing reviews to support safe and effective practice. Support prescribing safety activities, including high-risk drug monitoring, medicines safety alerts, and recall processes. Liaise with community pharmacies, secondary care, and other external services to ensure continuity of care following hospital discharge and outpatient appointments. Support repeat prescribing processes, ensuring timely, safe, and appropriate authorisation in line with practice protocols. Promote cost-effective prescribing and support achievement of local prescribing targets where appropriate. Maintain accurate and up-to-date clinical records in line with professional standards and information governance requirements. Participate in clinical meetings, training sessions, and multidisciplinary discussions as required. Maintain and develop clinical knowledge and competencies through continuing professional development and mandatory training.
