The post involves supporting adults who are recovering from illness and or trauma to recover and to gain independence through meaningful activity.
Customers can be admitted to Farmborough Court from hospital or their own home, and referrals are made via professionals within the City.
The service is fast paced as customers stay only a short time before being supported to move to either back to their own homes or to new accommodation depending on their needs. Our customers are often frail and frequently have more than one health condition, including varying degrees of physical and cognitive impairment. The care team works closely with our well established Multi-Disciplinary Team members including Social Workers, Therapists, Community Nurses, GP and Geriatrician.
The role of support worker whether day or overnight support is central to the delivery of excellent customer care and involves supporting the customer with all aspects of daily living routines including medication. This includes following therapy plans, and the use of innovative technology, for which training is also provided. The team prides itself on offering excellent customer experiences and support, at what can be a difficult time for customers and their family/carers.
This care and support should have the individual at its centre, promote respect, dignity, engagement, and participation, throughout the delivery of personalised care and support.
