Summary
The Urgent Care Service prevents avoidable hospital admissions
People unwell at home may require an urgent response or increased level of support to help them avoid a hospital visit. GP’s, and other healthcare professionals may refer to our service if they become concerned about your health and welfare.
We aim to respond to referrals within two hours and will arrange to visit to assess, diagnose, treat and implement carestrategies that will enable patients to remain safe and well in the comfort of their own home. In many cases, we are able to support individuals in avoiding numerous and sometimes distressing trips to A&E.
Alternatively, patients’ may be referred to the Urgent Care service following a discussion at a Single Point of AssessmentTeam (SPOA) an initiative aimed at integrating the Ambulance Service, Local hospital trusts and community teams within Kent. Where treatment pathways are agreed as an alternative to hospital attendance.

This role supports locality leads with the day‑to‑day running of the team and works closely with Caseload Coordinators to manage referrals into the Urgent Care Service (H@H and UCR). It involves monitoring team capacity, coordinating visits, managing incoming calls and queries, and escalating issues when required. The postholder undertakes basic triage using clinical algorithms, allocates work across the team, and supports clinical staff during daily handovers and weekly caseload reviews.
The role requires flexible working across East Kent within a multidisciplinary team and participation in a seven‑day shift pattern.
Responsibilities include creating and transferring confidential patient information within the Electronic PatientRecord system, adhering to Trust policies such as Information Governance, and contributing to KPI data collection.
Thepostholder also supports lone‑working safety procedures, maintains clinical equipment records, assists other teams when the Caseload Assistant is absent, and monitors service delivery, escalating concerns as needed.
