Summary
Key Responsibilities: Patient Care Support patients during medication initiation, titration and monitoring mental health and wellbeing Identify and escalate clinical risk to appropriate clinical leads, in line with protocols and in line with NICE guidelines Support patients with understanding their new diagnosis of ADHD, suggested treatment options, lifestyle adjustments and self-management strategies Care Coordination Work closely with the ADHD service delivery manager and care coordination team to support patient flow through the service and timely delivery of care Participate in MDT discussions, case reviews and clinical governance meetings Signpost patients to and liaise with other local services, including GP Surgeries, SPFT and any appropriate voluntary sector partners Record & System Management Maintain accurate and timely consultations within the clinical records using agreed clinical systems (full training will be provided) Contribute to outcome tracking, reporting and service evaluation activity as required. Service Development & Governance Contribute to the development of the service and clinical pathways to improve patient experience
