Summary

To work autonomously providing general medical services for Patients of Highcroft Surgery registered with us on a permanent, temporary or immediately necessary basis To deliver a safe, competent and acceptable standard of medical care, taking part in the day-to-day activities of Highcroft Surgery including surgery, paperwork and home visits where appropriate Maintaining appropriate records to ensure continuity and quality of care for Patients. These must be accurate, and where possible, avoid jargon to ensure Patients understand the content if viewing their clinical notes To collaborate with other GP Partners/employees and Practice staff of Highcroft Surgery on Practice development to achieve all clinical performance targets and directed or local enhanced services for Highcroft Surgery or primary Care Network (PCN) To take part in Practice Team leadership and take clinical responsibility and accountability for Practice population Working to promote Highcroft Surgery positively and engage with Patients in order to improve service delivery To be involved in Practice development, identifying service changes and developments in line with clinical needs locally. To develop these as part of the NHS Long-Term Plan and other national guidance Maintaining records of appointments, financial transactions and other administrative duties To maintain skills to the required professional level and undertake such training and development as may from time-to-time be required to maintain personal competency Be up to date with mandatory training and share evidence of this for compliance Participate in all activities required by CQC registration Have a zero tolerance to all forms of abuse To produce and comply with a work plan developed in partnership with the Practice Manager and Partners to ensure all Practice and other work is delivered appropriately and safely To undertake appropriate clinical risk assessments and work with the wider team to implement management strategies for risks identified Participate in activities required to assess compliance with CQC Fundamental Standards To provide high-quality medical services in the context of general practice to the Patients registered with Highcroft Surgery To provide a full range of services in line with the agreed requirements of the practice NHS contract including the quality and outcomes framework and any local and enhanced services To provide any services as required for the Primary Care Network agreement including shared QOF standards and service specifications. To provide o provide appropriate care and services to Patients in all age ranges and levels of health care needs To respond to medical/health problems presented by Patients on the registered list including history taking, diagnosis, investigation, treatment and referral as appropriate To provide management of long-term conditions in line with national guidelines and pathways To read and interpret test requests and results ordered by you and other team members taking appropriate action based on the findings To provide appropriate health promotion and preventative health care advice to all Patients attending the practice To prescribe generically for Patients as appropriate to need and be able to provide appropriate clinical rationale with support from the Medicines Management Team To carry out appropriate administrative and other duties in line with the needs of the Practice including referral to other services, maintenance of records, managing registers etc. To maintain clinical records either written or computer based in line with professional code and requirements utilising appropriate coding To work in line with appropriate Practice policies and procedures including human resources, safeguarding children etc. To provide appropriate health education and advice to Patients and carers to enable self-management and choice To undertake appropriate audit in practice to identify clinical quality issues and markers and define processes to improve clinical outcomes To take part in appropriate R&D and audit processes within Highcroft Surgery as agreed in line with primary care division priorities Assist management with maintaining compliance with agreed standards Promote and implement all the Practice policies and procedures In the course of your employment you will have access to confidential information relating to Highcroft Surgery and Patients. You are required to exercise due consideration in processing such information and you should maintain confidentiality for Highcroft Surgery and Patients The post holder will, if required to do so, obtain, process and/or use information in line with the GDPR requirements Participate in the assessment of the effectiveness of health care provided by the Practice, such as by auditing records Ensure that Patients rights are protected Promote and participate in a high standard of care within the Practice Develop and maintain effective working relationships with all other employees and contractors at Highcroft Surgery To manage the Practice caseload in partnership with other practitioners to ensure quality outcomes for Patient and carers To be actively involved in innovation To develop positive collaborative working relationships across all areas of the Care Team (primary, secondary and social care) and with carers To be actively involved and engaged in Highcroft Surgery and wider team meetings and forums to promote communication and development To provide on the day/triage for Patient and other services as agreed To liaise with all relevant agencies and services to promote knowledge and awareness of services in practice and local community To work with colleagues to identify ways to ease the burden of demand To identify ways to promote opportunities for Patient involvement in service development and evaluation To work flexibly in order to help cover leave, including holidays and short-term leave during sickness To deliver practice development projects for Highcroft Surgery as agreed with Practice staff, Managers and Partners or Business Owners To share and disseminate acquired knowledge with all Practice Team members through in-Practice developmental sessions and clinical governance meetings or significant event analysis (SEA) To work collaboratively to ensure that effective clinical governance is incorporated in the day-to-day practice To promote effective communication processes between all team members to enable good working practices To maintain communication processes with acute and other colleagues and partners in care to ensure care delivery To actively engage in effective communication with the ICB to ensure equity of service delivery and target achievement Co-operate in the effective resolution of team conflicts To provide appropriate medical leadership as appropriate to Practice requirements To identify and utilise best practice through benchmarking and clinical guidelines To take ownership and provide leadership to the Practice Team in specified clinical areas as part of Practice development programme To promote through leadership the use of end of life strategies to deliver high- quality palliative care To develop and implement as appropriate guidelines, policies and procedures as required To utilise the latest available evidence to shape and organise services to reflect needs of Practice population Co-operate in maintaining a staffing roster for providing adequate levels of care within the Practice Participate in developing the philosophy, goals and objectives for health care provided by the Practice Inform the Provider of any reported or suspected failings in the provision of care within the Practice using the significant event process To have knowledge of and always act within the boundaries of Health and Safety legislation, Data Protection Acts and Care Quality Commission compliance requirements Work within the GMC code of conduct Support and uphold an open, positive and honest working culture To undertake and complete appraisal process on yearly basis To undertake and complete GP personal appraisal to ensure revalidation is achieved To undertake appropriate training to meet personal and mandatory educational needs and practice development needs To undertake Practice/Personal development, in line with CPD objectives and record activity Give agreed notice of periods of absence and leave Attend Practice meetings as required Provide information for a DBS check, and inform the Practice of any incident that may change the DBS status Participate in taking care of your own Health & Safety. Ensure that you do not do anything to compromise the health and safety of either your colleagues or yourself
