Summary
This post provides a service to Calderdale and Huddersfield NHS Foundation Trust
The post holder is responsible for the management of a defined caseload to support patients with learning disabilities, and their carers access acute health services.
The post holder carries responsibility for the assessment of care needs, the coordination and implementation of complex care planning for patients with learning disabilities for unplanned, planned, day treatment and outpatient care including liaison with community services. The post holder will give advice for Autistic patients and support complex situations as per referral criteria for the team.
The post holder will support the Nurse Consultant in Learning disabilities deliver the Trust Learning Disability Strategy and other associated strategies ie; health equity etc
• Act as clinical role model, ensuring clinical expertise and practice is enhanced for patients with learning disabilities
• To clearly present the patients point of view to others, including promoting and protecting the interest and dignity of patients
• Establish and maintain effective communication within the multidisciplinary team
• To take an active role in risk assessment, supporting implementation of strategies to minimise risk.
• To recognise changes in patient conditions which require the intervention of others, and refer on as appropriate
• Act as an innovative and enthusiastic role model, providing leadership, guidance and advice to staff on operational & professional issues
• Make assessments and develop complex planning to enable a patient receive treatment. Act as the coordinator/participant within the multi-disciplinary teams responsible for care
• Maintain accurate records using the Trust’s documentation on EPR
Ensuring condition – related information is accessible to people with a learning disability and their carers.
• To identify barriers to accessing acute healthcare services for people with a learning disability and plan action and initiatives to overcome and facilitate easy access.
• In complex cases directly assist and facilitate the co-ordination of care at the following stages, preparation, attendance, admission and discharge.

• To take responsibility for own continuing professional development and performance, including identifying own development needs and maintain own portfolio in accordance with reregistration requirements
• To support the development of other team members and the wider workforce across the Trust as required,
• To manage verbal complaints and refer on when unable to resolve, encouraging junior staff to be proactive in the management of verbal / potential complaints
• To participate in investigation of incidents and complaints as required, including preparing detailed reports and letters to complainants, including triangulating the data to monitor for themes and trends
• Share lessons learned with the wider teams across the Trust to improve patient care for people with a learning disability.
• To recognise, manage and prevent areas of conflict, escalating where necessary
• Attend and participate in meetings as required, including providing feedback and disseminating information as appropriate
• Be proactive in the development of service development, through contribution of new ideas to improve patient care / experience for people with a learning disability across the Trust
• To have a high degree of budgetary awareness and support the ward/departments in the management of the budget and resources to have an overall impact in Trust financial position.
• Provide a lead and role model to nursing staff by delivering high quality care - Maintain a safe working environment
• Responsibilities for patient care - Assess, plan, implement and evaluate care and treatment using specialist knowledge and skills, advising patients and relatives as appropriate
• Organise own workload
• Receive clinical supervision from Nurse Consultant for Learning Disabilities
• Make assessments and develop complex planning to enable a patient receive treatment. Act as the coordinator/participant within the multi-disciplinary teams responsible for care
• Maintain accurate records using the Trust’s documentation on EPR
• Support patients and their families communicating sensitive condition related information and ensure they receive required information to enable them to participate in their care delivery
• Liaise with the multi-disciplinary team and Community Services (as appropriate) to ensure seamless provision of care delivery
• Provide training and education of learners and other junior staff
• Actively participate in the development of own personal development plan and performance review
• Provide education and training to patients and their carers as required
• Keep up to date with developments and advances within the speciality
• Contribute to Investigations and respond to complaints, untoward incidents and other significant events
• Work with the Nurse Consultant for Learning Disabilities to promote the development of Acute Liaison Nurse Specialist service for people with learning disabilities
• Under the direction of Nurse Consultant for Learning Disabilities participates in or leads the development of specific elements of practice, reflecting the Directorate and the Trust’s Nursing and Midwifery strategy and Learning Disability Strategy,
• Participate in the setting, monitoring and improvement of standards and the quality of patient care, including those standards defined in the National Benchmarks for Essential Care
• Promote evidence-based practice, wherever possible
• Engage users and carers views on service delivery and development
• Lead on patient and carer communication issues and coordination of the patient pathway for patients referred by the multi-disciplinary team
• Implement policy and practice guidelines specific to the speciality, proposing changes where appropriate
• Contribute to policy and protocol development for speciality area
• Exercise a personal duty of care in relation to equipment, resource and stock control utilising equipment and supplies appropriately
• Maintain record of own workload and performance monitoring information and review this on a regular basis with the lead nurse.
• Contribute to the research agenda
• Undertake audit programmes specific to client group, Participate in clinical trials as appropriate
• To facilitate and promote people with learning disabilities understanding of their own healthcare needs (overcoming barriers of cognitive perception/ reasoning)

• Provide and deliver raising awareness workshops and educational sessions to groups and individuals within the Acute Trust and other relevant areas.
• Provide advice and support regarding best interests and consent to service users, family, covers and professionals in accordance with the Mental Capacity Act (2005).
• Ensure that healthcare advice, support and information to clients and their carers is provided in accordance with their identified needs.
• Promote and facilitate the assessment of specific and holistic needs of patients, considering the additional needs of people with a learning disability who come from black and minority ethnic backgrounds.
• To provide a specialist point of contact for advice and signposting, assessing, developing and implementing specialist nursing care programmes, providing advice to people with learning disabilities, their carers and health and social care professionals
