CLCH Hertfordshire Adult Community Services- CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

Hertsmere Integrated Locality Team is a supportive, close-knit team, and we are passionate about delivering excellent care to our patients. We are looking for a Band 6 Nurse to join us as Deputy Team Lead, holding responsibility for a caseload of patients in the Bushey area. Please note that the Hertsmere Locality encompasses Potters Bar and surrounding villages, Bushey, Radlett, Shenley, Borehamwood and Elstree and there is an expectation that the successful post holder will work across these areas if there is a need.

Your role would be varied and interesting, delivering evidence based nursing care in people's own homes and in residential care environments. Nursing interventions include palliative care, medication management, wound care and pressure ulcer management, intravenous therapy administration and continence care. Evidence of transferable skills is essential for this role.

The successful applicant will be approachable, a good communicator, demonstrate strong leadership qualities, having the ability to work autonomously and be receptive to change. You will take a lead in growing and developing your Health Care Assistants, Nursing Associates and Community Nurses, as well as Students, so you must be willing to support others in clinical practice.

You must be able to drive and have your own vehicle.
• To set and maintain standards of care and deploy, co-ordinate and supervise the nursing team and learners as required.
• To provide leadership and management to a team of community nurses and other staff within the locality.
• To work cohesively with the wider multidisciplinary team as we build neighbourhood working connections.
• To be responsible for the team's assessment, planning, implementation and evaluation of individualised nursing care to patients living in the community. The service is provided seven days per week 8am-6pm. A separate twilight team and overnight team cover the extended hours services.
• To be responsible for ensuring your staff document appropriately, role modelling high standards of clinical documentation.
• To participate in the long term management of patients within the locality
• To have continued responsibility for a defined caseload
