Summary


To contribute to the assessment, implementation and delivery ofhigh-quality individualised client care within a professional team. To demonstrate responsibility to clients, relatives and colleagues through contribution to the development and monitoring of agreed evidence-based practice and to the health and social care programmes. To contribute to the clinical governance of theservice.

The Senior Support Worker will contribute to the assessment, implementation, and delivery of care within a multi-professional team.

Will demonstrate responsibility to clients, relatives, and colleagues through contribution to the development and monitoring of agreed evidence-based practice.

Will support the qualified multi-professional team and contribute to the implementation of health and social care programmes primarily within a client setting.

Will coordinate group activities, 1:1 activities and adherence to therapeutic care plans

Several of our service users are wheelchair users and there is a requirement to use hoists and push wheelchairs.

To ensure the service users personal information is always treated with the strictest of confidence. Any requests to disclose information must be relayed to senior staff.

Where required use basic Makaton with service users. To escort service users on site when required.
