Summary
To assist with general wellbeing; creating a supportive and re- enabling environment promoting maximum independence. The scope of the role will focus on supporting admission avoidance, early supported discharge and promotion of independence and assisting in the effective management of patients with long term conditions and Palliative patients. If candidates who are appointed do not hold the Care Certificate, an opportunity to complete will be available. The post holder MUST hold a driving licence with access to a car to fulfil this role. If successful this vacancy will require an enhanced DBS check at a cost of £54.90. This cost will be deducted from your first Trust salary In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.
