Crynodeb

To consult patients, face to face or remotely, to perform effective medication reviews, promote primary prevention and enhance management of chronic diseases under practice protocols and NICE guidance. Work with other initiatives including safe prescribing in cohorts such as the frail elderly and those with polypharmacy and opportunities for appropriate deprescribing. To provide guidance to the pharmacy and medicines management team. Working as part of a multi-disciplinary team in a patient-facing role, to provide primary support to general practice staff with regards to prescription and medication queries.
