Summary
Clinical To undertake annual health checks for all patients who have a learning disability across gtd primary care practices including a physical examination when required. To provide the service user with an annual health action plan. To support the service user to navigate services within primary care. To have an understanding of Quality Outcome Framework and use this to support annual health assessments. To act as a resource and point of contact providing advice, guidance and support for all gtd healthcare staff including trainee nursing associates when providing care for individuals with a learning disability. To facilitate the meeting of individual health care needs for people with learning disabilities, liaising with community learning disability teams to co-ordinate support for individuals with a learning disability who may require help. Raise the profile of the health care needs of people with a learning disability across care services to support improved communication. Contribute to the development of health care information and resources in accessible formats for both service users and their families / carers. Identify existing /potential barriers in accessing Primary Care services for people with a learning disability and promote initiatives to overcome these barriers. Support and enable gtd healthcare services to make reasonable adjustments within their care delivery for people with learning disabilities. Assist clinical areas in recognising potential barriers to such as communication issues in relation to individuals with learning disabilities, and provide advice and support as required. To undertake benchmarking and audit against recognised national standards. Promote the early resolution of issues and complaints working closely with the appropriate Manager and Clinical Leads. Recognise and work within own competence and professional code of conduct as regulated by the NMC/HPC etc. Understand own role and scope and identify how this may develop over time. Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures. Prioritise, organise and manage own workload in a manner that maintains and promotes quality. Ensure professional revalidation requirements are met. Deliver care according to NSF, NICE guidelines and evidence-based care. Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation. Support gtd healthcare and its workforce, including Trainee Nursing Associates, to comply with relevant mandatory, national, and statutory legislation, frameworks, and clinical guidelines. Liaise across Tameside, Manchester, and Buxton localities, working collaboratively with multi-agency partners to develop positive partnerships and improve outcomes for people with learning disabilities. Improve the quality of care received by individuals with a learning disability across Primary Care by providing advice, guidance, and support, and responding to concerns raised by clinical staff. Develop effective and collaborative working across primary care, providing specialist nursing advice related to the care of patients with learning disabilities. Provide advice and support relating to assessments for people with learning disabilities, including Mental Capacity Assessments and Best Interest decision-making in line with the Mental Capacity Act (2005). Assist with safeguarding enquiries, Learning Disability Mortality Reviews (LeDeR), and complaints involving people with learning disabilities. Work closely with the gtd healthcare Safeguarding Team and wider colleagues to develop, review, and update policies, guidance, and standard operating procedures that support people with learning disabilities. Ensure that all staff are supported and aware of the need to assess and implement reasonable adjustments for patients with learning disabilities. Managerial/Leadership To be a highly competent and knowledgeable registered health professional with experience in supporting people with a learning disability within primary care. To act as a role model within the field of learning disability care and inclusive practice. To ensure own clinical and theoretical knowledge and skills are maintained, remaining up to date with current developments, guidance, and best practice. To demonstrate an innovative and progressive approach to personal and professional development, underpinned by research and evidence-based practice. To participate in regular clinical supervision to receive professional support and promote reflective practice. To ensure full, accurate, and contemporaneous healthcare records are maintained in line with NMC record-keeping standards. To adhere to professional codes of conduct as set out by the NMC, gtd healthcare policies, and data protection requirements. To support and promote evidence-based practice, disseminate learning and research findings, and contribute to service improvement within safeguarding and the wider organisation. Education and Development To work in partnership with multi-professional/education staff to provide a comprehensive, innovative clinical skills and competency education framework. To work with the Head of Nursing, clinical leads, gtd academy and safeguarding leads, to develop a training needs analysis relating to learning disability requirements and development of teaching materials / training packages. To co-ordinate and assist in the delivery of training to meet identified neds specifically to ensure that colleagues are aware of and able to meet the needs of people with a learning disability. To contribute or deliver educational sessions to staff groups, liaising with other agencies and service uses in order to gain their participation in the education delivery. To act as a resource for staff in training posts, including Student Nurses and Trainee Nursing Associates, to support the development of learning disability awareness and competence across gtd healthcare. Clinical Governance Registered Practitioners have a responsibility to; Maintain current active professional registration Act always in accordance with their professional Code of Conduct and guiding documents. Adhere to Trust Policy and Procedure. Maintain up to date skills and knowledge and maintain awareness of professional issues. Maintain a professional portfolio.Non-Medical Prescribing If you are employed in a prescribing capacity, you must only prescribe within your scope of practice and comply with the gtd healthcare NMP governance registration processes. You must maintain your knowledge and skills and prescribe within your clinical competencies. The non-medical prescriber is required to comply with organisational NMP governance processes and adhere to local and national prescribing guidance. Apply infection-control measures within the organisation according to local and national guidelines. Managing Information Use technology and appropriate software as an aid to management in planning, implementation and monitoring of care, presenting and communicating information. Review and process data in order to ensure easy and accurate information retrieval for monitoring and audit processes. Other Duties: Any other duties, as agreed with the line manager to meet the needs of the organisation. This may include travel to other sites within the organisation.
