Summary

Main Duties/Key Results Areas: 1. Expert Clinical Practice: Work autonomously to review patients on the caseload whilst working as part of a multi-disciplinary team. Assess all newly referred patients and their families/carers, identifying their psychological, physiological, emotional, spiritual, social and cultural needs and co-ordinating specialist palliative care. Provide guidance, support, advice and counselling to patients and their family/carers along the patient pathway. Review patients independently and recommend changes in management of care. Attend weekly palliative care multidisciplinary team meeting and contribute to clinical case discussions. Attend cancer multidisciplinary team meetings as the core member for palliative care where requested. Provide clinical advice and support to all members of the multidisciplinary team in the care of patients with specialist palliative care needs, including ethical decision making, in accordance with current evidence base and London Cancer Alliance guidelines. Provide cover for the palliative care team at weekends/BH. Act as a role model of expert clinical practice, empowering others through effective role modelling, teaching and team working. Act as an expert practitioner in advising staff of the correct use and management of subcutaneous syringe pumps. Participate in case conferences in relation to future management or discharge arrangements. Act as patient advocate when relevant, ensuring patients are given adequate information to enable them to make informed choices and negotiating with the clinical team to agree a pathway of care. Ensure patients and their families/carers are given appropriate oral and written information regarding planned care and disease process. Provide support for distressed families/carers, identifying at risk families/carers and referring to appropriate agencies. Provide specialist advice and support on managing complex family and emotional situations and challenging behaviour. Provide early bereavement support to families/carers, liaising with bereavement services and referring to other skilled professionals where necessary. Liaise directly with key stakeholders in the patients care across all health and social care settings to ensure continuity of patient care. Develop guidelines and protocols to deliver a comprehensive, evidence-based specialist palliative care service. Recognise opportunities within the specialist palliative care service for further development and expansion, to facilitate and manage change within defined clinical and organisational boundaries. Work as an integral part of the multidisciplinary team to develop the service and appropriate referrals. Develop effective working relationships with all other departments and other regional and national organisations resulting in effective co-ordinated care for all clients. Set, monitor and review standards on a regular basis in accordance with clinical governance, essence of care benchmarks, NHS plan and NICE guidance. Prescribe/administer a number of drugs through the use of patient group directives, supplementary prescribing, or nurse prescribing. Lead on developing excellence in end-of-life care in liaison with members of the multidisciplinary team. The appropriate use of the Universal Care Plan record. 2. Education: Identify, assess, and meet the educational needs of the patient and their family/carers. Identify the educational needs of the multi-professional team and to facilitate and participate in education provision to meet those needs. Work collaboratively on educational strategies which support palliative care services and service development. Develop and deliver ward-based education programmes. Act as a mentor to nurses undertaking further studies in cancer/palliative care. Develop personal knowledge, skill, and expertise to a higher academic standard. Act as an expert resource working at a higher level in specialist areas. 3. Research/Audit Role: Have a good understanding of the value of quality research to improve patient care. Utilise up to date research/evidence to deliver patient care and to disseminate this knowledge to all members of the multi-professional team. Incorporate nationally agreed standards and guidelines into clinical practice. Act as a resource for colleagues undertaking research. Be aware of current research being conducted within own specialist area and be able to inform patients about these. Audit the specialist palliative care service on a regular basis so that patient care is continuously reviewed and improved. Ensure that documentation and record keeping is comprehensively maintained. Contribute to clinical governance developments in line with the strategic direction of the Trust. Share findings with a wider audience through publication and conference presentations. 4. Leadership and Consultancy: Provide a leadership role model within the clinical setting demonstrating expert knowledge and high standards of clinical practice. Demonstrate this higher level of practice at both local and network level. Provide line management to Band 6 CNSs where necessary with support from the Lead Nurse. Engage with both internal and external groups that will lead to the further development of the palliative care service and the Macmillan clinical nurse specialist role. Monitor and evaluate ones own service implementing new initiatives where needed for further patient benefit. Contribute to the development of clinical nurse specialist services across the Trust, through liaison with the CNS forum and other clinical nurse specialists. Contribute expertise to Trust and network wide policy development and review, within specialist area. Contribute to the development of cancer services across the Trust through liaison with the Macmillan lead cancer nurse, cancer teams and other Macmillan specialists. Disseminate evidence of service development to other Macmillan nurse specialists. Work alongside local and national networks and partnerships promoting and disseminating evidence of service development. Work closely with newly qualified clinical nurse specialists to assist them in their professional development. To contribute positively to the leadership of nursing within the care group acting as an effective role model. To comply with measures to actively manage and reduce risk to patients. Ensuring the formal reporting and recording of adverse incidents in line with Trust policy. To utilise and maintain information systems to aid audit and to provide regular activity analysis reports. Maintain contemporaneous and accurate treatment records, submitting relevant statistics, reports and activity data as requested. Ensure that confidentiality of information is adhered to and NMC standards for Record Keeping are met. To participate in the evaluation of the service in terms of clinical effectiveness, clinical excellence and value for money. To ensure the best use of available resources is used within agreed budget to provide a cost effective service. Contribute to the local induction of staff within the care group, as appropriate. 5. Professional Role: To act in accordance with the NMC Code of Professional Conduct for Nurses Midwives and Health Visitors and Scope of Practice and be accountable for his/her actions at all times. To achieve a range of clinical competencies as required to fulfil the role. To keep up-to-date with current literature and research in the speciality. To maintain your own personal and professional development in accordance with PREP requirements, attending mandatory study sessions as required. To undergo and actively participate in own performance appraisals. To adhere to the NMC Code of Professional conduct working within and accepting responsibility for maintaining agreed levels of competence. To be working towards higher degree or equivalent (if not already achieved). To have responsibility for the Health, Safety and Welfare of self and others and to comply at all times with the requirement of the Health and Safety Regulations.
