Summary
To support and lead multidisciplinary teams to deliver an efficient holistic palliative care service, that can respond flexibly to patient and carer needs and is focused on achieving agreed outcomes for the patient To manage and inspire a multidisciplinary workforce from novice to expertto work as one team To actand practiceas a senior clinician within the community teams and, as required, the wider organisation To innovate and streamline community processes and services by embedding quality improvement methodologyincluding clear outcome measures to produce efficient and highly performing community services It is anticipated thatthe role willincludeclinical work in your set discipline (20%), management and leadership(80%) including supporting/deputising for the Head of Community Services when required
