Summary
You will form an integral part of the surgery and responsible for your list of patients, including the administration of documents, results, referrals. You will have sound understanding of local pathways and the referral processes , training for the local pathways will be provided as part of the induction process. You will form part of our duty system that means you are available for home visit triage and visits, supported but our PCN Acute Home Visiting Service. You will work collaboratively within our multidisciplinary team to deliver high quality, patient-focussed healthcare. Colleagues requiring Sponsership are welcome to apply.
