Summary

To undertake the assessment of patients and clients with complex care needs including individuals with undifferentiated and undiagnosed life limiting conditions. To plan, implement and evaluate specialised programmes of care utilising evidencebase to achieve optimum best clinical practice in palliative care and supportive care. To provide specialist advice to staff in managing complex situations, ensuring that the patients/clients needs are met and that significant others are supported. To independently liaise and communicate effectively with all members of the multidisciplinary team and other agencies involved in the care of the patient/client. To initiate risk assessment processes to determine risks to the health and well-being of patients with complex care needs, taking relevant action to minimise these risks. To promote patient/service user involvement in the planning, delivery and evaluation of care, respecting their wishes, beliefs and dignity. To utilise information and data from a range of sources to identify patient/client issues and guide the decision-making processes. To have an active role in education of HSC and outside agencies charities staff through in-service training, multi-disciplinary training programmes, student training and work experience. To ensure that professional standards are upheld by the staff members working within the postholders sphere of responsibility. To preserve safety by following and contributing to the development of policies, procedures & guidelines relating specifically to Palliative care nursing. To be accountable for ensuring that safeguards are in place for patients/service users and escalate any safety, safeguarding or risk concerns following the appropriate processes
