Summary
We’re looking for a compassionate, organised Transition and Placement Coordinator to support adults with complex needs as they move into our specialist Day Service. You’ll coordinate personalised transitions and placements, liaise with families, carers, and external agencies, and work closely with a multidisciplinary team to ensure every individual thrives.
