Summary

The Post Holder will work as part of the memory service team and provide high quality clinical assessments, care and treatment for people experiencing memory problems. Following completion of appropriate training leading to a recordable Independent Nurse Prescribing Qualification, the post holder will practice as an independent non-medical prescriber in accordance with; the Trusts non-medical prescribing policy and The Code for nurses and midwives (NMC). This job description is an outline of the main duties of the post. The postholder will be required to undertake other duties commensurate with the grade as directed. The content of this post will be reviewed in consultation with the postholder when necessary and in line with ongoing service developments. KEY RESULT AREAS: The postholder will: 1.1 Care/service/function provision: Develop therapeutic relationships in which people using services are empowered and enabled to make informed choices about their health and healthcare. Participate in; the management of referrals, provision of clinical supervision to memory service nurse colleagues and junior clinical staff facilitating case review discussions when required. Provide comprehensive mental health and social care assessments and provide care, treatment and educational interventions within the bounds of the agreed protocol and/or pathway within the service. Review the efficacy of anti-dementia medication that has been prescribed for service users in line with the service pathway & protocol. Act as care co-ordinator for service users as required. Attend multidisciplinary meetings and care programme meetings as required. Ensure that carers needs are assessed and appropriate interventions or onward referral is provided depending on need. Communicate assessment outcomes sensitively to service users and relevant others. Inform the referring clinician of the assessment outcome and plan of care and treatment through provision of effective clinical reports. Promote health through provision of health education information and advice Following qualification as an independent non-medical prescriber, prescribe medication as an independent nurse prescriber within their own sphere of competence and the bounds of their approved prescribing status in accordance with the Trust non-medical prescribing policy. Promote patient choice and understanding when communicating assessment outcomes or prescribing medicines, including provision of high quality information both written and verbal to support this. Be responsible for ensuring the security and safe handling of FP10 prescription forms and to report any loss or theft as per Trust guidelines 1.2 Working as part of a team: Recognise the limits of their own expertise and (in line with the local protocol/ pathway for assessment of suspected dementia), request additional assessments from more senior multi-professional colleagues when required. Access regular caseload and clinical supervision (as identified within the local service protocol/pathway), to summarise, present and discuss assessments and therapeutic interventions that have been completed with more senior multi-professional colleagues when required. Utilise an appropriate range of multi-agency and inter-professional resources in their work, liaising effectively with other teams and agencies according to individual service user/ carer need. Work collaboratively in partnership with service users and health & social care professionals to provide specific interventions which are evidence based and appropriate with regard to context and culture. Liaise effectively with colleagues from primary care and non- memory specialist secondary services, providing information and advice where appropriate on memory assessment and interventions. Actively support the teams development of prescribing protocols, policies etc. as required. Ensure that the patients GP is notified of any changes to that patients care or treatment, including commencement of any new medication in accordance with Trust policy. In the event of adverse drug reaction, to be accountable for ensuring that the GP/Consultant is advised accordingly. Appropriately liaise with staff from external health, social care and third sector organisations. Maintain effective working relationships with colleagues both within and outside the employing organisation Participate in relevant research projects as required 1.3 Managing Self: Maintain effective and ongoing continual professional development relevant to their role and in accordance with their appraisal, trust policy and NMC validation requirements. Maintain accurate high quality contemporaneous health records in line with current Trust policy and professional body requirements. Maintain electronic records on the relevant medical record systems (RIO) where necessary Undertake clinical assessments which are; comprehensive, person centred, manage risk and are appropriate to needs, context and culture. This will incorporate use of appropriately validated psychological & psychometric screening and assessment tools. Maintain competency as an independent nurse prescriber in accordance with the requirements of the Trust non-medical prescribing policy and NMC standards. Maintain competency in memory assessment, through ongoing professional development and in accordance with NMC revalidation requirements. Ensure that appropriate records are made in the patients RIO notes which identify the postholder as the independent prescriber Promote clinical excellence by complying with agreed Trust and statutory policies and clinical guidance, while maintaining own clinical competencies in this role Work to the required standards of the The Code for nurses and midwives (NMC), at all times. Ensure fulfilment of requirements for continuing professional development as required by professional body, eg. NMC and as outlined in the Trust non-medical prescribing policy Attend and provide training events as necessary to maintain knowledge/skills and competence of self and others Work within local and national agreed guidance for best practice Work at all times only within their sphere of competence Maintain appropriate indemnity insurance through a professional body Will be expected to travel between sites to attend clinics and meetings including duties commensurate with the post holders grade as agreed with the line manager 1.4 Management Supervision and Support To be responsible for the supervision, support and guidance of Band 5 Nurses and Mental Health Support Workers where appropriate. Participate in a formal appraisal programme and provide support for lower banded staff Contribute to clinical audit and clinical governance activities as directed by the team manager/ nurse consultant Participate in relevant forums and networks for non-medical prescribers to support and update on service development/legislative changes and pharmaceutical updates Participate in the Trusts appraisal performance reviews of self and team members Participate in and undertake clinical supervision for junior members of the team within the service on a regular basis Participate in appropriate teaching and training for service staff and students. Ensuring attendance at mandatory training Work with the wider team, participating in service quality monitoring and evaluation, including the requirements of the Memory Service National Accreditation Programme requirements. Provide advice and support to other lower banded staff in relation to conditions and treatment plans, based on medical/drug history and knowledge of potential contraindications.
