Summary
To work within the community nursing service to support patients with diabetes, as part of a team collaboration with Wirral University Teaching Hospitals diabetes service. The post-holder will have managerial and clinical supervision from WCHC, with access to specialist clinical supervision and professional development from the Wirral University Teaching Hospital team This work will include: Supporting discharges from hospital for patients with complex insulin regimens as part of a multidisciplinary team, ensuring safe, timely and well-coordinated discharges. Developing and delivering training and advice in diabetes and diabetes management to staff in and beyond district nursing, including titration of patients on insulin. Providing direct specialist diabetes nursing assessment and support (including for self-management) to their own caseload of patients, typically housebound, and including those on palliative care pathways, ensuring their diabetes management needs are met. To conduct joint home visits and to provide specialist clinical patient assessments, to support frontline clinical staff in the delivery of high quality, safe, integrated community care. To contribute to the development of best practice and facilitate shared learning to develop services and initiatives that meet local needs, in relation to aims and objectives of Wirral Community Health and Care NHS Foundation Trust. To support the development of the pathway to support a self-care model for patients with diabetes on the community nursing caseload within the Nursing Directorate.
