Summary
Intermediate care incorporates several functions and aims to support patients through their pathway whether it is in the hospital or the community. Therapists also provide rehabilitation services to Trinity Ward at Ripon Community Hospital and Station View in Harrogate. Assistant Practitioners will have an essential role within the multidisciplinary The intermediate care team, enabling people to stay at home and maintain/increase their independence by working across health and social care boundaries, and provide rehabilitation and care between 8am and 8pm. In general terms, therapists work with people to help them overcome the effects of illness, injury or accident and enable them to lead full and satisfying lives as independently as possible. They also work to promote health and well-being and empower patients to manage their own conditions.
