More detail about the role The post holder will be predominantly based within the acute heart failure team at North Devon District Hospital, providing specialist HF care for inpatients but there is opportunity for outpatient, telephone and community follow up too. As the service is an integrated heart failure service, the post holder will be asked to work flexibly across sites and different elements of service provision. The role encompasses diagnosis, symptom management, optimising treatment, supporting patient self-monitoring, education, and delivery of effective palliative care. There is a huge emphasis on multi-disciplinary working and the post holder will benefit from taking part in cross site clinical multi-disciplinary meetings.
