Summary
Actively track patients urgently referred to the Trust from General Practitioners (GP) or General Dental Practitioners (GDPs) who are suspected at referral of having Cancer. Ensure that these patients are listed on the Trust Cancer IT system. Ensure that appropriate requests for investigations are initiated following the clinicians instructions and that appointments are given To adhere to the Trusts General Principles and Guidelines on Patient Access in relation to the management of waiting lists. To remove non-malignant patients from the Cancer pathway by assessing oncology notes, Somerset, PAS, RIS and ICE and to work with medical secretaries and the clinical team to identify such patients in a timely manner Ensure that any newly diagnosed cancer patients are identified and added to the Trust Cancer IT system. Ensure that the patient is known to the clinical team and that appropriate action is taken to manage the patient on the agreed pathway. Highlight any potential delays or breaches of the patient pathway according to the Trust escalation policy To liaise with the members of the MDT both within the relevant Directorate and across the Trust as a whole to ensure coordination of all elements of the patient journey. This may include highlighting patient target dates etc to ensure that investigations are carried out and results received in a timely manner to ensure that the patient reaches the point of treatment within relevant time-scales. To ensure all information relating to patients on a cancer pathway clearly identifies all relevant target dates to aid other members of the MDT in allocating resource appropriately To be aware of availability of staff and events which may impact on the patient pathway (eg consultant leave, MDT meeting cancelled due to bank holidays) and make allowances for this in the patient pathways.
