Description
We are seeking a dedicated and compassionate CSW - Social Worker - Localities (Gr 8) to join our team. In this pivotal role, you will work within the community to provide essential support and services to individuals and families facing various challenges. Your expertise in social work will enable you to deliver personalized care, working closely with clients to assess their needs, develop tailored intervention plans, and connect them with appropriate resources. Responsibilities
Conduct comprehensive assessments of clients' social, emotional, and economic needs.
Develop and implement individualized service plans that address clients' challenges and goals.
Provide direct services including counseling, crisis intervention, and advocacy for clients.
Collaborate with community resources and agencies to facilitate access to services and support.
Monitor and evaluate clients' progress toward achieving their objectives and adjust plans as necessary.
Maintain accurate and timely documentation of client interactions and service delivery.
Participate in interdisciplinary team meetings to share insights and coordinate care for mutual clients.
Requirements
A master's degree in social work from an accredited institution.
Current state licensure as a clinical social worker or eligibility for licensure.
At least three years of experience in social work practice, preferably in community settings.
Strong knowledge of social service systems, resources, and community services.
Proven skills in assessment, intervention planning, and crisis management.
Excellent communication and interpersonal skills to effectively engage clients and collaborate with teams.
Cultural competence and a commitment to diversity, equity, and inclusion in practice.
