Summary
We will be operating aclinical triage modelfrom February 2026 to ensure timely access to appointments. As part of your role, you will participate in the triage rota, making clinical decisions and prioritizing appointment requests via eConsult. Outside of triage duties, you will manage your own clinics, providing care for acute, complex, and long-term conditions. You will be responsible for delivering a safe and efficient patient journey, including all associated administrative tasks such as referrals, reviewing test results, and completing documentation. You will also be expected to take part in the Home visits and care home rounds as required. You will be mentored and supported by the Partners and wider Multi-disciplinary team, which included Management, Admin, Nurses, HCAs Social Prescribers, Pharmacists. You will be expected to take part in Practice meetings revelnt to your role.
