Don't miss out on this rare opportunity to join an established team of Highly Specialised Diabetes Nurses in our Mid-Nottinghamshire Team.
Our Diabetes Nurses work with primary care colleagues to provide the highest quality proactive care to our patients in Mid-Nottinghamshire. Working as part of the Diabetes LES (Local Enhanced Service) in Nottinghamshire.
You will work as part of a system to provide the best care possible as part of this innovative primary care facing enhanced service, that has training and development at the heart of its delivery.
You will work in predominantly clinic settings with the Newark PCN area to provide LES clinics with our primary care colleagues.
You will help develop the skills in primary care.
You will be supported by your peers and colleagues in secondary care to provide the highest possible level of care.
You will work with our community nursing colleagues and wider MDT to help ensure that patients in the community are optimised.
You will be a key part of a highly successful team.

This is a predominantly clinic based role with is mostly primary care facing working with GPs and Practice Nurses to provide the highest possible care to patients.
You will also be part of the wider local integrated team - which is neighbourhood focused.
Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.
You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.
A full UK driving licence and vehicle for business use is required for this post; however, reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
