Summary
The discharge co-ordinator will facilitate timely, safe, and effective patient discharge through close collaboration with multidisciplinary teams and external agencies. Participate in board rounds and MDT meetings to ensure clear, accurate discharge plans documented in PPM+. Communicate sensitively with patients, families, and professionals to provide up-to-date information on discharge pathways. Monitor patient flow, identify delays, and escalate issues promptly to support seamless transitions of care. Maintain accurate records of discharge status, waiting times, and reasons for delay, ensuring compliance with Trust policies. Promote teamwork, guide less experienced colleagues, and uphold confidentiality, safeguarding, and health & safety standards while adapting flexibly to service demands.
