Your role:
This role is an essential part of delivering some of our services ambitions around improving customers health, wellbeing and lifestyle, tackling health inequalities experienced by customers which leads to early mortality rates, particularly across our learning disability services.
You will lead on the development of programmes which employ an asset-based approach whilst enabling individuals to have improved health outcomes and develop new assets and strengths. Working with teams and individuals across in-house and external services, providing training to develop knowledge and skills regarding health, nutrition to improve activity levels, achieve the best outcomes regarding physical & mental health improvements.
You will connect with relevant health professionals to provide specialist support and advice which ensures individuals health and wellbeing is a priority.
About you:
You will have a background in supporting individuals to make life changes which improves their health and wellbeing for the long term. You will be a creative, passionate, and driven individual who is committed to the people you work with and supporting to achieve their goals and ambitions.
You will have excellent communication skills with the ability to develop strong working relationships with customers, families, champions and other relevant professionals.
You will have experience of being part of and leading projects, working in partnership with key stakeholders to continuously improve services for our residents.
You will be highly motivated and proactive with a flexible attitude.
