Summary
The Practice Nurse will play a key role in delivering high-quality, patient-centred healthcare services. Core responsibilities include: Support the Infection Prevention and Control (IPC) Lead and work in accordance with the organisations IPC policies and procedures. Undertake comprehensive patient assessments through consultation with patients and, where appropriate, their families, to identify health needs and develop appropriate care plans. Support the management of long-term conditions across the organisation, contributing to quality improvement, clinical audit, and assurance initiatives. Deliver specialist respiratory care with a focus on asthma and chronic obstructive pulmonary disease (COPD), supporting high-quality long-term condition management. Undertake structured annual asthma and COPD reviews in line with national and local clinical guidance and best practice standards. Perform and interpret fractional exhaled nitric oxide (FeNO) testing to support diagnosis, monitoring, and optimisation of respiratory treatment. Undertake diagnostic and monitoring spirometry, ensuring compliance with recognised quality standards, accreditation requirements, and infection prevention protocols. Develop, review, and regularly update personalised respiratory management plans in partnership with patients to promote self-management and improve health outcomes.
