To support the BCUHB in increasing the number of patients undertaking self-care and home based dialysis. To train and support patients to perform a range of tasks relating to their dialysis according to individual ability. To implement a training program that will change the culture of staff and patient education and define the nursing and medical role to emphasise patient empowerment and self-care. To enhance education of the medical condition including the role of dialysis, medication, diet and oral fluid restriction, as well as the impact of the illness on quality of life. To provide a consistent approach and share best practice across BCUHB.
