Developing an excellent organization To support the organisation's ways of working and model its values Key accountabilities Act as a point of contact for the Home Visiting Service, taking telephone calls / requests for visits from practices and other health and social care partners / and ensuring these are logged in the patient record and directed to a Clinician when appropriate in a timely manner. Following up on any outstanding actions. Deal with telephone/email (individual and service generic email) enquiries, taking messages and passing information to the team. Monitor and manage SystmOne tasks. Assist the shift lead in the day-to-day operations of the Home Visiting Service list and assist with allocation of visits. Keep the visiting staff informed of any matters arising as necessary Effectively communicate with patients and GP practices around forthcoming home visits and keep patients informed of any changes. Be a point of contact for the Home Visiting Service to patients, the Practices and other health and social care services using the service. Ensure that all contacts/visits undertaken are recorded correctly in the medical records. Support with contacting patients for feedback and devise patient feedback reports. Preparing agendas, taking minutes, distributing notes of meetings and chase and coordinate actions and updating action logs. Participate in any Home Visiting Service development activities as directed. Provide support with service data collection to support PCN board reports and other communications as necessary. Support the Clinical Leads, Clinical Operations Manager and PCN Operational Lead as required. Responsible for ordering stationery and consumables for the team, ensuring they are kept securely. Carrying out stock takes, making sure stock is in date. Assisting with data capture and coding on a range of systems; using GP clinical records and systems as well as other non-clinical administration IT systems. Work flexibly to ensure coverage during periods of absence within the team. To carry out other appropriate delegated duties as required. This is an evolving role, but could be subject to change as the impact of primary care network commissioning evolves
