Crynodeb
Job Responsibilities To respond to the general nursing requirements of practice patients. Be responsible for meeting the nursing care needs of patients identified by the nurse, or where care has been transferred to the nurse by the GP. In accordance with the NMC formulate individual care plans and evaluate outcomes at appropriate levels according to need. To evaluate the outcome of that care, making changes to the care plan for individual patients where appropriate. To demonstrate the rationale behind the various methods chosen to meet care needs of patients, to evaluate those methods and change them as necessary. Review and plan care for patients with long term conditions.
