Summary
Act as a single point of access to patients who are being registered to use My Medical Record as part of their care Proactively support and coordinate for the overall needs of patients who are on a digital virtual care pathway using own judgement and escalating to the relevant practitioner as necessary Support and coordination of patients on virtual pathways Please note that this is a 5 day service
