Summary
To maintain a personal and professional profile in line with the Post Registration Educational Practice (PREP). To participate as appropriate pre-admission and other assessments as directed by the needs of the service. To facilitate and participate in the development and maintenance of individual care plans, undertaking practical aspects of patients care to which may include working on the ward. To attend and participate in clinical team reviews, Care Programme Approach and risk management meetings. To promote and maintain good working relationships and effective communication systems within service users, carers, colleagues and external agencies. To promote and maintain both personally, and within the Care Team, a high level of standards, understanding and adhering to local and national policies, procedures and guidelines ensuring innovative and safe practice. To participate in the assessment, development and maintenance of individual care/treatment plans including risk assessment/management plans for individual service users. To assist and participate in the co-ordination and maintenance of an accurate records system. To actively participate in audit and research projects in order to advance the boundaries of care. To provide and participate in a system of regular supervision, providing support and professional advice when required, including encouragement of personal and professional development.
