Core Responsibilities: The Practice Nurse Manager will be responsible for, but not limited to: Clinical Leadership and Management: Lead and manage the nursing team, including nurses, nurse associates, and healthcare assistants. Co-ordinate the nursing team rota to meet patient needs and ensure adequate staffing. Delegate clinical responsibilities appropriately, ensuring tasks are within the scope of practice of staff. Support staff through appraisals, training needs analyses, mentoring, and guidance. Plan, implement, and manage changes within the nursing team to enhance service provision. Infection Prevention and Control (IPC): Act as the IPC lead, working to the IPC policy and providing audits, training, support, and evidence of compliance. Produce the IPC annual statement in conjunction with the practice manager. Clinical Duties: Provide routine and acute nursing care in accordance with clinical evidence, NICE guidelines, and the NSF. Implement and evaluate treatment plans for chronic disease patients, supporting health and wellbeing. Identify, manage, and support patients at risk of long-term conditions. Diagnose and manage acute and chronic conditions. Prescribe medication in line with PGDs and scope of practice, reviewing patient medications to enhance compliance. Provide guidance on prescribed and over-the-counter medications. Oversee wound care services and chronic disease clinics, referring patients to secondary care when required. Coordinate travel medicine services and implement vaccination programmes for adults and children. Request and process pathology tests and results. Develop, implement, and participate in well woman and well man clinics. Chaperone patients and assist GPs with minor surgery when required. Support the team in dealing with clinical emergencies. Recognise, assess, and refer patients presenting with mental health needs. Support patients with safe medication use and liaise with external services for vulnerable patients. Support safeguarding across the practice in line with national and local policies. Recognise substance misuse issues and refer appropriately. Deliver opportunistic health promotion where appropriate. Governance and Quality: Maintain accurate clinical records and ensure effective use of read codes. Ensure appropriate maintenance and use of chronic disease registers. Review and implement nursing protocols in line with national guidance and local policy. Participate in audits, quality improvement projects, and shared learning within the practice. Review significant or near-miss events using structured approaches, such as root cause analysis (RCA). Ensure the nursing team operates in line with legislation, policies, and practice procedures. Service Development: Develop, implement, and embed health promotion and wellbeing programmes. Participate in local initiatives to enhance service delivery and patient care. Continually review clinical practices, responding to national policies and initiatives.
