Summary

Clinical Practice Provide specialist nursing practice intervention so that the specific needs of the individual patients/clients are addressed. Provide direct care as per job plan, working alongside the nursing staff in the clinical setting and providing a visible strong role model to ensure best practice. To provide advice and support for all members of the multi-disciplinary team in the care of patients with a stoma at all stages of their pathway. To communicate effectively with members of the multidisciplinary team to ensure patients with a stoma experience seamless, high quality service. To provide guidance, support, advice and counselling to patients and their family/carer. To recognise personal limitations and know when to refer patients to a specialised counselling service. Education To be accountable for own professional practice and responsible for acquiring, developing and maintaining competencies and skills at an advanced level commensurate to this role. To identify, assess and meet the educational needs of the patients and their families. To actively teach nursing staff in the clinical area and evaluate their learning in conjunction with the senior nursing staff. To work collaboratively on education strategies which support stoma services and service development. Be regarded as an expert resource in stoma care. Leadership To provide a leadership role model within the clinical setting demonstrating expert knowledge and high standards of clinical practice. Empower other colleagues with their own professional development. To be highly visible, accessible and approachable to staff, patients, carers, families and the public, ensuring that channels of communication are sustained and created. To contribute to the development of patient pathways, policies and protocols, to enable changes in practice to be achieved, implemented and evaluated. Research and Audit To have a good understanding of quality research to improve patient care. To contribute to audit to evaluate the effectiveness of the service on a regular basis and disseminate to the multidisciplinary team as required. To contribute to clinical governance developments in line with the strategic direction of the Trust. Collect and support the analysis of information related to patient satisfaction and evaluation of the service.
