Summary
Job Summary The post holder will work as part of our team to provide high-quality care to the patients of our 37 practices across our Federation. The Health Care Assistant will work autonomously to deliver safe and effective nursing care. Previous primary care experience is essential. Key Responsibilities Phlebotomy Wound care and dressings NHS Health Checks and other preventive health activity Chronic disease monitoring and reviews within scope of competence Clinical Duties Undertake venepuncture and ensure high standards of practice across the Federations phlebotomy service. Deliver wound assessment and management, including dressing changes and post-operative care. Including removal of clips and sutures Conduct NHS Health Checks and contribute to proactive health promotion initiatives. Undertake chronic disease reviews (e.g., hypertension, diabetes, COPD, asthma) in line with protocols and within scope of competence. Accurately record clinical data on the relevant clinical systems (EMIS). Escalate concerns or clinical issues appropriately to the supervising Lead Nurse or GP. Recognise deterioration in wounds and refer onto the complex wound care clinic as necessary Professional Responsibilities Work within NICS protocols and procedures Work with scope of practice and recognise when to seek advice from colleagues Maintain accurate, contemporaneous, and confidential patient records. Adhere to infection prevention and control policies and procedures. Communication and Team Working Communicate effectively and compassionately with patients, families, and colleagues. Collaborate with NICS colleagues to support integrated patient care. Champion our NICS services with practices and PCNs including attendance at meetings where appropriate and valuable.
