Summary

About us
The Review Team is located within the Vale of Glamorgan’s Longer Term Care Service which provides support to individuals and their carers who are in receipt of care and support. The Review Team is responsible for completing both scheduled and unscheduled reviews to evaluate progress towards achieving personal outcomes, establishing that any managed care and support arrangements remain eligible for local authority support, and completing re-assessments where required. The Review Team also supports other service areas as they arise.

Previous experience of working with Adults is desirable but not essential

About the role
Pay Details: Grade 6 scp 14-19 £29,540-£32,061 Pro Rata £15.31 - £16.62 Per Hour
Hours of Work: 37 hour of the week
Main Place of Work: Canolfan Ty Jenner, Barry

Responsible for completing both scheduled and unscheduled reviews to evaluate progress towards achieving personal outcomes, establishing that any managed care and support arrangements remain eligible for local authority support, and completing re-assessments where required.

About you
What you need:

Experience of working in a Team;
Experience of working with people in a paid, caring role or voluntary capacity
Some knowledge/ understanding of ageing/illness/disability and its effects of individuals/families and their Carers
Good standard of written and verbal communication skills. Ability to organise and prioritise tasks. Negotiation skills
Ability to work on an inter-agency basis.

Ability to work as part of a team. Ability to relate to and empathize with people.
Creative and imaginative approach to problem solving.
Ability to work under pressure
Willingness to be supervised
Willingness to change work location as necessary
