Summary
Care Home & Housebound Services Deliver proactive and reactive care to residents in nursing and residential homes Support structured care home rounds and urgent reviews Contribute to anticipatory care planning, frailty management, and end-of-life care Work closely with care home staff to provide clinical advice and guidance Multidisciplinary Working Work collaboratively with GPs, Practice staff, ARRS roles, community nursing teams, and social care Participate in MDT meetings and care planning discussions Maintain effective communication with patients, carers, and families Governance & Quality Maintain accurate clinical records using GP clinical systems Adhere to PCN and practice policies, clinical governance, and safeguarding procedures Contribute to service development and quality improvement initiatives across the PCN
