We have an exciting opportunity for a Community Support Worker to join our Community Rehabilitation Transition Service!This role is not eligible for sponsorship as is does not meet the requirements as set out by the UK Visa and Immigration Service.About the ServiceOur Community Rehabilitation Transition Service provides clinical and practical support to people who are ready to live more independently but still require significant input from our nursing and community care teams to remain safe and well in the community.The service also supports people transitioning from community-based support or stepping down from hospital into 24/7 care, with clear and structured plans in place to ensure safe and successful transitions.Our teams are based within our nursing services and are supported by our wider community care teams. This approach allows individuals to maintain established therapeutic relationships with colleagues they know and trust, while being supported to move towards more independent living.About the RoleAs a Community Support Worker, you will support individuals with complex mental health needs to live independently in their own homes as part of the Community Rehabilitation Transition Service. Working under the guidance of the Community Clinical Lead and Service Managers, you will deliver person-centred, recovery-focused support, promoting independence and working collaboratively with service users, carers, and the multidisciplinary team to achieve positive outcomes.Due to the nature of this role, it is an essential requirement that you hold a full UK driving license and have access to a car. You will Support qualified staff with assessments, support planning, and risk management for new service users.Assist in delivering treatment and support plans under clinical supervision.Build positive, therapeutic relationships with service users and their carers.Provide practical and emotional support, including ADLs, to promote independence.Act as a keyworker when required, supporting coordinated and consistent care.Encourage community engagement as part of rehabilitation and transition.Maintain accurate records on electronic systems (e.g. RIO).Work collaboratively with the MDT, external professionals, service users, and families.Uphold confidentiality, contribute to team communication, and engage in training and development.What we're looking forWe would love to hear from you if you possess the following: Experience supporting people with complex mental health needs and a good understanding of mental health conditions and interventions.Awareness of safeguarding responsibilities and person-centred practice.Strong communication and organisational skills, with the ability to work both independently and as part of a team.A flexible, professional, and motivated approach, with a genuine commitment to supporting this client group.Respect for individual rights, dignity, confidentiality, and autonomy.Willingness to develop skills and engage in continuous professional development.Competent IT skills (e.g. MS Office and CRM systems).Alignment with and commitment to the organisation’s values.What we offer in returnWe are proud to have been recognised and certified as a Great Place to Work, which speaks volumes on how much we value our staff members.
