Summary

The main aim of the role will be to support patients in the region with complex FND in accessing and navigating care. 1. Clinical and liaison responsibilities Act as first point of contact for selected patients with FND, providing advice, education and practical support in understanding and managing their condition. Offer specialist liaison input to acute hospitals across South East London, assisting in the development of care plans to prevent avoidable admissions and reduce length of stay. Provide case management for a caseload of complex community patients requiring low-intensity, containment-focused support, minimising iatrogenic harm and ensuring continuity of care Facilitate MDT case discussions and contribute to borough-based FND multidisciplinary meetings. Access clinical support and guidance from an identified senior colleague of the same professional background as needed. 2. Education and training Deliver training and advice to acute and community staff on recognition and management of FND, informed by evidence-based practice and models developed within the FND ExSEL Network Develop and disseminate written guidance and patient information resources. Engage in ongoing professional development activities to maintain and develop clinical skills in line with the post holders discipline, with organisational support. 3. Communication and Coordination Maintain effective communication between treating teams, ensuring that patients and carers understand diagnosis and management plans Liaise with GPs and community services to coordinate ongoing care and prevent duplication Record and maintain accurate patient information, including emergency care plans and agreed contact points Coordinate with other FND services within the SEL network when providing occasional cover or cross-service support.
