A fantastic opportunity has arisen for a motivated Physiotherapist or Occupational Therapist to join our highly skilled and experienced Early Intervention Team at Cambridge University Hospitals.

We are looking for an innovative and motivated team player, working collaboratively with other members of the multidisciplinary team, including our Community partners, to provide a holistic, patient-centered admission avoidance service.

You will support speedy discharges for patients with long term conditions and complex health needs. You will provide a specialised therapy assessment in order to prevent the unnecessary admissions of patients into acute care or to begin their acute rehabilitation.

You will be responsible for providing a high standard of assessment, advice, treatment and evaluation of patients to formulate treatment plans for patients. This may include patients with diverse presentations and complex physical and psychological conditions.

The role will allow development of specialist knowledge of ED and Frailty specific assessments, with an opportunity to evaluate practice and audit outcomes. Development opportunities will include continuing professional development, links with Community Services, contribution to operational group meetings and striving to meet our national and local guidelines.

You will be expected to contribute to seven-day working between the hours of 8am and 8pm, based on a rota system.
• Responsible for the provision of high quality, patient focused care.
• Responsible for assessing, developing and implementing specialist therapy provision in the Emergency Department, SDEC areas and acute Frailty Unit.
• Responsible for providing specialist advice and support to, and liaising with, other staff to support admission avoidance and early supported discharge where appropriate.
