Summary
Assessing new patients for specialist mental health diagnosis when indicated. Drafting treatment plans in a bio psychosocial model Working closely with primary care / GP/ Voluntary sector / Social prescriber Regular liaison with the SMHT area consultant for patients requiring ongoing care coordination or secondary care treatment for severe mental health conditions Offer advice for the team when needed Arrangement and review of investigations when indicated. Audit and service development. Contribution to management in team meetings. Attending the MDT
