Our Sensory Services team have a unique focus, supporting adults born with sensory impairments, or acquired at any age. Many of the people the team work with also have lifelong physical disabilities, learning disabilities, autism, and brain injuries acquired at an early age. We also work with adults with sensory impairments acquired at any age (people who are Deaf, Deafblind, or who are visually impaired). The team offers a dedicated, strengths-based approach to disability that we know makes a positive difference. Whether we are engaging with people seeking advice, providing equipment or skills training, or starting a journey with people that may lead to a personal budget to meet complex needs, we are always looking to deliver the best outcomes.

About The Job
Salary: £29,540 to £32,061 per annum
Working pattern: 37 hours per week
Contract type: Permanent
Locations: Office base location Horsham. Hybrid working opportunities will be discusses at interview, with an expectation of 2 days per week in the office.
Are you passionate about making a difference? Our Assistant Care Managers are an integral part of the countywide team also made up of Vision Rehabilitation Specialists, Senior Practitioner Vision Rehabilitation Specialists, and Social Workers for deaf and hard and hearing and visually impaired people.
Our Assistant care Managers need to be resilient, adaptable, organised and able to work in a fast-paced environment. You will be expected to learn new and adaptive ways of communicating with people with sensory impairments, and promote sensory needs within the Council and with our partners.
In this role, you will:
- Use your skills and knowledge to develop and implement tailored support plans that improve wellbeing and deliver positive outcomes.
- Provide advice, information, the provision of assistive equipment or independent living skills.
- Visit people in their own homes and undertake high-quality risk assessments and reviews, including Mental Capacity Act assessments.
- Respond to safeguarding enquiries and work collaboratively as part of a dedicated team.
- Ensure compliance with organisational policies and legislative requirements.
- Participate in an out of hours duty system
- As part of the role is visiting people in their own homes you will be required to travel independently throughout the County including to locations that are not always easily
accessible by public transport.

Experience and Skills
The following criteria needs to be covered, with examples where possible, within your application form:
Key Skills:
1. Ability to understand the impact of disability, and in particular sensory impairment on a person's wellbeing.
2. Ability to complete a strengths-based assessment (as directed by the Care Act 2014).
3. Ability to develop a person-centred care and support plan with clearly defined outcomes.
4. Excellent communication and interpersonal skills, including the ability to use a variety of information technologies.
5. Ability to prioritise and organise work, meet deadlines and agreed performance expectations.
Qualifications:
* A minimum of 3 GCSEs including English and Maths or equivalent qualification, e.g. NVQ.
* NVQ Level 3 in Care - Desirable
* Experience of providing support/working with people with disabilities / sensory impairments.
* Experience of forming effective working relationships within a multi-agency setting.
* Experience of providing advice and guidance to others.
* Experience of applying statutory interventions that prevent, reduce or delay the need for ongoing support.
* Experience of applying relevant legislation, policies and procedures to individual pieces of work.
