Summary
Job Summary The Practice Nurse will work as part of the primary healthcare team to deliver high-quality, patient-centred nursing care to the practice population. The role includes managing long-term conditions, health promotion, preventative care, and treatment room duties in line with evidence-based practice and national guidelines. As a Welsh-speaking practice, the ability to communicate confidently in Welsh is essential to ensure safe, effective care for our patients. Key Responsibilities Clinical Responsibilities Provide nursing care including wound care, immunisations, cervical screening, and treatment room duties Undertake long-term condition management clinics (e.g. asthma, diabetes, COPD, hypertension) Carry out clinical assessments, monitoring, and reviews in line with practice protocols Promote health and wellbeing through patient education and preventative care Maintain accurate and timely clinical records using the clinical system Work within professional scope of practice and recognise limitations, referring appropriately. Communication. Communicate effectively with patients, families, and the multidisciplinary team Support continuity of care and build positive, trusting relationships with patients Infection Prevention & Safeguarding Adhere to infection prevention and control policies Participate in safeguarding adults and children in line with legislation and practice procedures Quality & Governance Contribute to quality improvement, audit, and clinical governance activities Maintain professional registration and engage in continuing professional development Follow practice policies, procedures, and national guidelines Team Working Work collaboratively with GPs, Practice Manager ,the Physician Associate, nursing, dispensing, and administrative staff. Support a positive team culture within a small, communicative practice Professional Responsibilities Maintain NMC registration and revalidation requirements Act in accordance with the NMC Code of Professional Conduct Take responsibility for own learning and development Participate in training and appraisals. Confidentiality The post holder must maintain patient confidentiality at all times in accordance with GDPR, Caldicott principles, and practice policies. Other Duties The post holder may be required to undertake other reasonable duties appropriate to the role, in line with service needs.
