UNLOCK YOUR BEST WORK LIFE + MAKE A DIFFERENCE TO EVERY LIFE We have an exciting opportunity for a Release and Transfer Lead to join our Primary Care team at HMP Brixton name, a category B/C Prison located in Brixton. HMP Brixton is a male prison with a capacity for around 800 inmates. In order to meet the needs of patients in our prisons, Practice Plus Group have developed a new integrated approach to the provision of care. Teams will work together at each stage of the patient journey, supporting them as they arrive in the prison (Early Days in Custody Team), delivering the care they need, promoting good health and supporting resettlement (Planned Care Team) and responding to urgent and emergency needs (Unscheduled Care Team). You do not need to be clinical or have a registration for this role. Hours Monday to Wednesday 0900 - 1700 Salary This position is full time, youll receive an annual salary up to £61,119 per annum FTE - depending on experience. How will we support you?
