Summary

1. Ensure provision of chemotherapy for haemato-oncology patients as well as other day ward based treatments such as venesection and central venous access devices management. There is an expectation that the post-holder will spend time working clinically in the day unit. 2. Have oversight of chemocare for Haematology patients to ensure timely scheduling. 3. Act as a Specialty Advisor giving evidence based advice to all staff in matters relating to the care of patients with haematological disease. 4. Lead the nurse management of patients with acute/chronic haemato-oncology disease. 5. Act as key worker for patients with haematological disease. 6. Provide evidence based information and advice for patients and their carers on all aspects of acute and chronic haematological disease. 7. Co-ordination of the patient pathway and a core member of the multidisciplinary team. 8. Promote effective communication with nursing team, patients, relatives and other health care professionals. 9. Due to the clinical nature of the role close collaboration with the Cancer Services Pharmacist Team is essential. 10. Support the AOS service 11. To provide expert nursing advice to patients with Haematological malignancies and their families throughout the course of their illness from diagnosis, assessing their needs and providing information as required. 12. To liaise with all relevant members of the multidisciplinary team in the care and support of individual patients including GPs. 13. To maintain a high profile in the clinical area, acting as a role model for other health care professionals 14. Where appropriate hold a nurse led clinic including the development of written protocols based on national standards 15. To offer advice and support on pain and symptom control liaising with the Specialist palliative care team, and make appropriate referrals to the chronic pain service. 16. To assess individual patients psychosocial and spiritual needs and refer for appropriate counselling or to other agencies 17. To help co-ordinate weekly multi-professional meetings for Haematology cancer patients to achieve high standards of MDT working . Responsible for implementing the prersonailsed care program for Haematology patients including completing of HNAs assisting with health and well-being events and treatment summaries where appropriate Education / leadership 1. To promote patient/carer independence through the provision of relevant teaching and support. 2. To participate in and organise study days/education programmes to raise awareness of the holistic needs of patients. 3. To develop resource and teaching packs for use by patients and/or staff in the clinical and educational setting. 4. To ensure the provision of appropriate written information is available to patients and staff 5. To participate in the Trusts ongoing educational programmes. 6. To evaluate the need for patient support groups within the Trust. 7. Liaise with other charities, organisations and agencies dedicated to cancer ensuring patients, their families and carers are informed of the availability of existing support services. Research/Audit/ Quality 1. To utilise up-to-date research findings in the delivery of patient care and to disseminate this knowledge to all members of the multidisciplinary team. 2. To participate as appropriate in relevant research studies and audit findings and disseminate these findings to clinical practice as appropriate in order to develop best practice. 3. To identify opportunities for nursing research within own specialist area and carry out nursing research appropriate to the speciality. 4. To regularly audit service including eliciting the views of patients, carers and professionals ensuring patient feedback is incorporated into developments in the service. 5. To attend relevant departmental and Directorate and network meetings relating to clinical governance and to contribute to the clinical governance agenda. Management 1. To liaise with other members of the team to ensure that there is an efficient use of resources, human, financial and time. 2. To ensure that there is a safe and acceptable environment for patients. 3. To act as a patient/staff advocate through the application of ethical, legal and professional knowledge and skills. Personal development 1. To maintain own expert level of competence. 2. To participate in Annual review with the Clinical Nurse Lead 3. To critically reflect on own performance through clinical supervision, coaching or mentoring. 4. To work within the NMC Code of Professional Conduct and all Trust Guidelines. 5. To continue own personal development through the pursuit of relevant professional and academic study. 6. Participate in Annual job planning 7. Prepare for NMC revalidation Collaboration/ Consultancy 1. To work in a collaborative manner with all members of the multidisciplinary team recognising the unique contribution of other disciplines and appropriately referring. 2. To recognise own limitations and seek advice and support on referrals. 3. To act as a resource within the Trust and to participate in Trust-wide professional groups as appropriate. 4. To represent the Trust locally and nationally. 5. To work in a collaborative manner with local primary care teams. 6. To provide specialist advice and support related to the purchasing and commissioning of resources/services. 7. To act in an advisory/ collaborative role with senior management in issues relation to the specialty. 8. To act as a specialist resource for the management of patient
