We are looking for a compassionate and motivated Reablement Enabler to support people in their own homes to regain independence and confidence. This is a rewarding role focused on short-term, goal-based support rather than long-term care.

You will provide practical and personal support tailored to each individual, helping them rebuild everyday skills following illness, injury, or hospital discharge. Duties may include supporting with personal care, assisting with household tasks, and encouraging participation in activity or exercise programmes designed by healthcare professionals.

Support is delivered in a positive, enabling way, empowering individuals to do as much as possible for themselves. You will play a key role in motivating people, monitoring progress, and celebrating achievements as they work towards agreed outcomes.

The role involves close collaboration with a multidisciplinary team, including therapists, nurses, and social care professionals, ensuring care is delivered safely and effectively. You will be responsible for maintaining accurate records, reporting changes in needs, and contributing to reviews of progress.

This position suits someone who is empathetic, reliable, and confident working independently in the community, with a strong commitment to promoting dignity, choice, and independence. You will report to a Reablement Senior Enabler or Coordinator and receive guidance and support to develop within the role.
