The Health care support worker will form an integral part of the Plastic Surgery Clinical Nurse Specialist (CNS) team in caring for the physical, social and psychological needs of patients diagnosed with cancer, requiring plastic and reconstructive surgery and their carers throughout their cancer care and following treatment.
You will provide essential administration support to the CNS team by triaging phone calls, supporting with completing and sending of referrals, managing appointments for nurse led clinics, liaising with community teams, updating, and maintaining databases.

Triage telephone calls to the CNS team phone liaising directly with patients/carers, signposting working with defined protocols and guidelines and escalating as needed to CNS and clinical team. Documenting and communicating outcomes.
Document and monitor all aspects of patient care coordination and service delivery.
Ensure patient safety, quality, and experience across whole pathway.
Acting as a liaison and point of contact between primary and secondary care.
Support the CNS within team with admin and carry out administration duties as required for role.
To build good relationships with Trust colleagues including MDT Leads, Clinicians, MDT coordinators and other Departmental Heads.
Competent and proficient with using a computer and various software applications including Microsoft office: Outlook, Excel and Word.
