Do you want to make a difference to someone’s ability to remain safe and independent in their own homes?
Do you have experienceof working in the community as a HCA, reablement support worker, Rehabilitation support worker, Therapy assistant, Therapy Technician.
Do you enjoy working in Lutoncommunity providing a prevention and rapid intervention service for people who have fallen or at risk of falls?
The service works shifts from 8pm until 6pm, 7 days per week with weekend work on a rota basis. A typical shift pattern for a full time member of staff may include 4 working days over a 7-day period with 3 rest days.
Due to the nature of the role it is essential that applicants are able to travel effectively and independently on a daily basisto and from the base, clients’ homes etc whilst transporting equipment. If necessary, adjustments can be considered, in accordance with the Disability Discrimination Act.

We have an exciting opportunity for motivated, well organised and enthusiastic individuals to join our falls team as Community Falls Support worker. You will be based in Luton as part of a small proactive team but will be able to work autonomously using your own initiative, have good communication skills and be IT literate.We offer the successful applicants a variety of interesting work in a friendly and supportive team.Training will be provided and the Trust is committed to staff development.
To respond quickly to all referrals for patients who have fallen or at risk of falling, arranging to visit them as soon as possible.
To visit patients in their own home or place of residence.
To complete a comprehensive, evidence-based falls assessment and identify any equipment, additional care or support required to enable the person to remain at home safely.
To support the Luton Falls Team in delivering all aspects of the falls strength and balance classes/ group programme as necessary to include: patient reviews, exercise assessment, education and psychological support.

To support delivery of all elements of the falls strength and balance group program: namely- face to face groups, virtual sessions both live and pre-recorded, and home exercise sessions.
To assess for, provide and demonstrate basic mobility equipment and equipment to aid activities of daily living.
Based on the falls assessment, to make recommendations to patients and their carers that will reduce the risk of further falls.
To make referrals to other services where this will reduce the risk of further falls and promote patients’ independence.
To follow up patients to ensure that equipment has been delivered and care is in place.
To maintain and update patient records in a timely manner using an electronic system and to work closely with the Falls Coordinator to collate and analyse associated data.
To undertake related administrative duties.

To analyse facts and make judgements within own competency level. This will involve the use of skills to evaluate the patient’s condition and adapt the assessment and recommendations as necessary.
To work with service users and their carers to design packages of care that meet assessed needs, promote independence and choice, respect service users’ dignity and rights and meet cultural needs. To communicate information to the relevant services to support the smooth implementation of care.
To assess for, provide and demonstrate basic mobility equipment and equipment to aid activities of daily living.

To undertake related administrative duties – completing notes and onward referrals in timely manner to Social services, safeguarding, Community rehab, Assistive Technology Services, Age concern, Total Wellbeing, Fire Service, Medequip and other specialist services with CCS.
To be responsible for planning and prioritising own workload.
To work autonomously according to service protocols, procedures and pathways.
To contribute to the development of efficient operational procedures and local pathways and participate in service development projects and initiatives. To comment on proposals for changes in own area.
The post holder may be required to organise and run group activities.
To support team Lead in completing Audits.
To support team in promotion activities within Luton through other service partners.
