Key Responsibilities Provide generalist and specialist palliative care across community and practice settings. Lead anticipatory care planning, including advance care planning and DNACPR discussions. Support symptom management, medication optimisation, and reducing avoidable hospital admissions. Act as a clinical lead, supporting colleagues and contributing to service development and quality improvement. Full details of all responsibilities are provided in the Job Description. Why Join Us?
